Am I Eligible for funding?
People living with disability (under 65 years, or under 50 for ATSI) may be eligible for government funding to help pay for support services . To check your eligibility please call Central Coast Community Pathways on 1300 160 096.
The National Disability Insurance Scheme (NDIS) supports eligible people under 65 who have a diagnosed disability. Click on the link to find out more about the scheme, the eligibility criteria and the types of support available here: http://www.ndis.gov.au/
Frail older people (65 and over or 50 and over for ATSI) may be eligible for a Home Care Package if they are assessed by the Regional Assessment Service (via My Aged Care) as requiring hostel level care but are able to continue to live in their own homes.
Your first point of contact is My Aged Care – please phone 1800 200 422 or visit www.myagedcare.gov.au. They will conduct a short assessment over the phone, and arrange for an in-home assessment by the Regional Assessment Team who operate in your local area.
If you’re eligible for subsidised in-home care, you can ‘top up’ with extra support at any time – simply call us on 1300 578 478 and we can arrange this fee-for-service support as a one-off, or ongoing if you need it.
If you’re a Department of Veterans’ Affairs client (DVA), you must be referred by your doctor. Services offered by Adssi HomeLiving Australia to DVA clients include Occupational Therapy (OT) and Home Modifications and Maintenance:
Occupational Therapy – our registered and qualified Occupational Therapists provide in-home assessments with the aim of improving your safety and independence both at home and in the community.
The Occupational Therapist will make their recommendations in consultation with you and/or your carer. DVA approval is required for assistive equipment, continence products, installation of grab rails or bathroom modifications. Our OTs manage this approval process on your behalf.
Home Modifications and Maintenance – we are a preferred contractor to DVA approved service providers, ie Aidacare, Mondial, Paraquad and Henrycare.
Services can include: Installation of ramps, rails and other modifications that help you access and live in your home more safely (DVA clients require an OT assessment before modifications can begin)
Handyperson services for small home maintenance and repairs such as minor carpentry, electrical, plumbing and hose reels
Major bathroom modifications and the installation of lifts
